Online appointment

Dear patients, you can use the form below to make an appointment with us online.

It's very easy!

1. First select the reason for your visit.
2. Then select your preferred doctor.
3. Then select a preferred day and time.
4. You will also need to give us your name, date of birth, email address and whether you have public or private insurance. (The fields will open one at a time so that you cannot accidentally enter the wrong information).
5. Finally, we need your current cell phone number so that we can send you a confirmation text message.
6. In the end, our Dr. Flex scheduling system will send you an email confirming the dates you have selected. (This email will also include buttons to reschedule or cancel your appointment).
